Appeal to Reinstate a Schedule Cancelled for Nonpayment of Tuition

• In accordance with Florida State University Regulation 5.081 Tuition, Fees, Payment, students who do not pay tuition and fees or make arrangements to pay tuition and fees by the end of the established fee payment deadline may have their schedules cancelled. Change Your Legal Name of Record

To change your official, legal name of record with the university, please email two forms of documentation to the Office of the University Registrar at registrar@fsu.edu. Acceptable forms of documentation include driver licenses, passports, and court orders. The name change will be processed within one business day, and it will be reflected on all future official FSU documents, including certifications, transcripts, and diplomas.

Note: We cannot accept marriage certificates unless the document specifically indicates that your name has legally changed.

• The State Employee Tuition Waiver provides full-time State of Florida employees with the opportunity to take up to six hours of coursework per academic term at a significant discount. The program was established per F.S. 1009.265 State Employee Fee Waivers.
